American Journal of Cardiologyに採択されたG Di Pietroの論文「Systematic Review and Meta-analysis of Short-Term Outcomes in Patients Following Protected High-Risk PCI」に対するcommentaryです。

その論文に対し「Impellaは心原性ショック（CS）やハイリスクPCIに用いられる重要な補助循環デバイスである。Pietroらは外科不能例でのハイリスクPCIにおける短期成績の有用性を報告したが、適応拡大にはRCTと登録研究が必要である。また、今後ハイリスクPCIの定義標準化、臨床・解剖学的因子による層別化、コスト評価が今後の課題とされる。ImpellaはIABPより良好な傾向を示すが、出血・AVWSなど合併症への対策が重要であり、個別化治療が求められる。」とコメントをしました。
